GENERAL SUMMARY:  The Jacksonville Port Authority (JAXPORT) is seeking a qualified and licensed firm to serve as the BENEFITS AGENT OF RECORD FOR DENTAL AND VISION PLANS for approximately 150 full-time employees. The selected vendor shall serve as the Agent of Record on behalf of JAXPORT for the Dental and Vision Benefit Plans.  Firms must provide professional advisory and administrative support services, including but not limited to, General Management of the Dental and Vision Benefits, Procurement and Renewal Support, Enrollment and Administration Support, and Reporting of Trends in association with the best practices in dental and vision benefits.  JAXPORT intends to award a contract to the lowest, most responsive and responsible firm based on the proposed Carrier-Paid Commission Percentage, who will be the designated Prime Bidder and the single point of contact and fully responsible for providing these services as per the specifications, terms and conditions set forth in this RFQ. Therefore, the purpose of this Request for Quote (RFQ) is to request information about your abilities to provide these services exactly as specified and to obtain a pricing commitment.
